Hormonal Fluctuations and Alcohol’s Amplifying Effect
During menopause, the ovaries gradually produce less estrogen and progesterone, which play a major role in regulating everything from mood to metabolism. According to the North American Menopause Society (NAMS), alcohol can worsen hormonal symptoms, particularly night sweats, hot flashes, and sleep disturbances (NAMS, 2022). These effects are more pronounced in midlife women due to alcohol’s influence on body temperature regulation and neurotransmitter balance.
Dr. Lauren Streicher, a clinical professor of obstetrics and gynecology at Northwestern University, explains: “Alcohol acts as a vasodilator and can trigger vasomotor symptoms like flushing or intense warmth, especially in women already experiencing hot flashes. Even moderate drinking can disrupt thermoregulation during menopause.”
Sleep, Mood, and Cognitive Function
Many women use a glass of wine to “wind down,” but alcohol may actually disrupt sleep architecture. A 2019 study published in Menopause, the official journal of NAMS, found that perimenopausal and menopausal women who consumed alcohol before bed experienced lower-quality sleep and more nighttime awakenings. This happens because alcohol interferes with REM cycles and increases cortisol—a stress hormone already in flux during midlife.
Alcohol is also a central nervous system depressant, which can amplify symptoms of anxiety or depression in some menopausal women. Dr. Judith Joseph, a board-certified psychiatrist and women’s mental health advocate, notes that “while alcohol may provide temporary relief from stress, it can worsen mood swings, reduce emotional resilience, and increase the likelihood of depressive episodes over time.”
Bone Density and Cardiovascular Risk
Postmenopausal women are already at increased risk for osteoporosis and cardiovascular disease due to declining estrogen levels. Alcohol adds to both risks. Research published in the Journal of Bone and Mineral Research (2017) found that excessive alcohol intake was associated with lower bone mineral density and higher fracture rates in postmenopausal women.
On the cardiovascular side, while some earlier studies suggested that red wine in moderation could have heart-protective benefits, more recent evidence suggests that any alcohol use raises the risk of atrial fibrillation, hypertension, and stroke in midlife women (World Health Organization, 2022). The American Heart Association no longer promotes alcohol as a heart-health strategy, particularly for those over 50.
How Much Is Too Much? Moderation, Not Elimination
So, does all this mean women should give up alcohol completely during menopause? Not necessarily—but moderation is crucial. The Centers for Disease Control and Prevention (CDC) defines moderate drinking for women as up to one drink per day, but this may still be too much for women experiencing heightened menopausal symptoms. It’s also worth noting that alcohol affects women more strongly due to lower water content and slower metabolic breakdown compared to men.
Dr. Sharon Malone, an OB-GYN and menopause specialist, recommends limiting alcohol to special occasions or switching to lower-alcohol beverages like wine spritzers or mocktails. “Listen to your body,” she advises. “If alcohol makes you feel worse—physically or emotionally—it’s okay to take a break or stop entirely. Your wellbeing in this phase is worth protecting.”

Whether you reduce or eliminate alcohol during menopause, support your body with nutrition, movement, and sleep hygiene. Focus on foods rich in calcium, magnesium, and omega-3s.
